Medical Record Destruction Notice

Peak One  Surgery has a retention period for medical records of 10 years past the last encounter date.  Minor medical records are kept for the longer of (a) Last service date  + 10 years, (b) Age of majority + 10 years.  With permission granted from the Division of State Archives and Public Records, all medical records falling under the above timeline will be destroyed.   Anyone wishing to request their original medical records must contact Peak One  Surgery Center a minimum of 3 months prior to the 10 year anniversary date.  Please call 970-668-1458 between 9 a.m. and 5 p.m. Monday through Friday to request a form.   Standard release of medical records charges may apply for additional electronic records.
